Using insurance

In-network with several major plans.

Coverage varies by plan. Before your first appointment, call the member-services number on your insurance card and ask about outpatient mental-health benefits, your deductible, and your expected copay or coinsurance.

Helpful question to ask:
“Is Anita Cooper-Marquez, LMFT, in-network with my specific plan?”

Carelon Behavioral Health
Blue Shield of California
Providence Health
Headway: After fit is confirmed, clients with select insurance plans may receive instructions for scheduling and plan management through Headway. The initial consultation is still requested directly with Anita.
Out-of-network: If your plan is not listed, Anita can provide a monthly superbill for you to submit for possible reimbursement. Reimbursement is determined by your plan and is not guaranteed.

Good Faith Estimate

Information you have a right to receive.

If you are not using insurance, federal law may entitle you to a Good Faith Estimate explaining the expected cost of your care before services begin. The cash rate is also included in the informed-consent documents provided before treatment.

For official information, visit CMS.gov/no-surprises. Questions about how this applies to your situation can be discussed with Anita.
